Use this tool to prepare the form to ask a court to make an accommodation for you if you have a disability. You can ask a court to accommodate your needs so that you can fully participate in a court activity such as a hearing, jury duty, mediation, or some other event in a court. You will go to the LawHelp Interactive website to do this. When you are done answering the questions, your finished form will be ready to print.
